Preparing for Your Appointment
To ensure the best care and a smooth registration process, please bring the following items to your first appointment:
Identification
A valid government-issued photo ID (Driver's license, Passport, etc.)
Insurance Cards
All current medical insurance cards and any necessary referral forms.
Medication List
A list of all current medications, including dosages and frequency.
Insurance & Billing
Allied Urology participates with most major insurance carriers, including Medicare and Medicaid. However, insurance coverage can vary significantly between plans.
- Verify Coverage: Please contact your insurance provider before your visit to verify that our physicians are in-network.
- Co-payments: Co-pays and deductibles are due at the time of service.
- Referrals: If your insurance requires a primary care referral, please ensure it is processed before your arrival.
Your Privacy Rights
We are committed to protecting your medical information. In accordance with the Health Insurance Portability and Accountability Act (HIPAA), you have the right to:
- Inspect and copy your protected health information.
- Request an amendment to your medical records if you believe they are incorrect.
- Request restricted use or disclosure of your information.
- Receive an accounting of disclosures we have made of your health information.
