Insurance Cards

All current medical insurance cards and any necessary referral forms.

Medication List

A list of all current medications, including dosages and frequency.

Identity Protection: To comply with federal regulations and protect you from identity theft, we require photo ID and may ask security questions when you contact us by phone.

Insurance & Billing

Allied Urology participates with most major insurance carriers, including Medicare and Medicaid. However, insurance coverage can vary significantly between plans.
